Full-time Renal Population Health Nurse (RN)

Position Summary: This position is directly involved in a collaborative process of assessment, planning, facilitation, and advocacy for options and services to meet a patient’s renal health needs across settings that span the continuum of care (i.e., post hospitalization, skilled nursing, acute rehab and home, hemodialysis). Acute care needs are identified through assessment, risk of readmission, data reporting and collaboration with hospital discharge planners, primary care physicians and advanced practice providers (APPs). The Renal Population Health Nurse supports the team in achieving clinical standards across the continuum of hospital, clinic and community settings, coordinates resources, and provides acute and renal nursing care.

Responsibilities include but are not limited to:

  • Remote but must reside in CNY
  • Develops and updates the individualized care plan based on a comprehensive assessment based upon the patient and/or family/caregiver, their Nephrologist and the patient’s primary care provider.
  • Engage community resources that are timely and cost-effective to obtain optimum value for the patient in support of medical and social determinates of health.
  • Provides education/coaching in self-management of the renal disease processes.
  • Monitors the patient’s medication adherence in conjunction with the Family Care Medical Group Pharmacist and Nephrologist.
  • Conducts on-going communication and collaboration with the patient and/or family/caregiver, as well as the patient’s Nephrologist, Primary Care Provider and other significant health care providers.
  • Conducts, at minimal, monthly monitoring of the patient’s health, safety, and functional status, and progress toward established renal goals.
  • Conducts in-home visits to renal patients as needed (5%-10% of the position).

Required Qualifications:

  • Registered Nurse (RN) with active New York State License – BSN preferred but experience considered in lieu of
  • 5 years minimum in nursing and 3 years case management preferred

Diabetes/Endocrinology Nurse Practitioner

FamilyCare Medical Group, PC, a multispecialty physician group, is in search of a Diabetes/Endocrinology Nurse Practitioner who will function as part of a multidisciplinary team of providers and will be given considerable autonomy to provide high-quality comprehensive care in the outpatient setting, focusing on patients with diabetes.

About the position:

  • NP will provide direct care to adult patients with type 1 and type 2 diabetes, weight management, prescribe diabetic medications and titrate insulin, provide patient education on     diabetes and prediabetes as well as glucometer and insulin use. Will manage use of insulin pumps, continuous glucose sensors and related diabetes technology.
  • NP will provide direct clinical care for the patient population according to established protocols and utilize evidence-based practice standards
  • NP will perform approved therapeutic or diagnostic procedures based on the patient’s clinical status
  • NP will address patient and family concerns and create a therapeutic relationship to provide support. Will provide patient education, counseling, and instruction regarding patient diagnosis, medical treatments, medications, and preventative care, as well as identify patient/family need for community resources and refers them appropriately.
