Palliative Care Team

  • Elizabeth Olsen, RN

    Registered Nurse with FCMG Palliative Care Program. Nursing career spanning 28 years with experience in health promotion, primary care triage, cardiac ICU, obstetrics, ambulatory surgery, and chronic disease management. Also served as Health Education faculty of Wilson Family Practice Residency Program in Johnson City, NY. 

    Educational background: Double major Bachelor’s of Arts in Biology and Psychology from Boston University. Bachelor’s of Science in Nursing from Binghamton University. Certificates in Paramedic Studies, Diabetes Education, and Clinical Research Coordination. Graduate studies in Health Education at SUNY Cortland, and One Health and Infectious Disease at University of Glasgow, Scotland. 

    Interests and hobbies: Currently involved in research of vector-borne, neglected tropical diseases in the United States. Active volunteer with Cornell University Farmworker program, Rotary International as a Youth Exchange Officer, and Medecins Sans Frontieres New York Office. Enjoys traveling with her husband and four sons, as well as cooking, painting, and gardening.

     

  • Bridget Schoeneck, MSN, FNP-C, ACHPN

    Bridget works in the home-based palliative program, seeing patients in the home for palliative consults and follow up. She has worked in Palliative Care for 9 years, both in Massachusetts and New York state. She has experience working with hospice as well. Her most recent job was on the inpatient palliative team at SUNY Upstate, focusing mostly at the Community campus. She left that at the opportunity to bring Palliative Care to the homes in our community, where it's most needed. She is nationally certified as an advanced hospice and palliative nurse.

  • Marijo DiMora, MSN, NP-C, ACHPN

    Marijo DiMora is a certified nurse practitioner in family health who started working with the outpatient palliative care team through FamilyCare Medical Group in February of 2024. She started her nursing career at Upstate University Hospital after graduating from Cayuga Community College in 2014 with her Associate Degree in Nursing and later completed her Bachelor of Science in Nursing degree through Roberts Wesleyan College in 2016. Her background in nursing includes care of inpatient medicine patients with her later years focused on women's health, functioning as a labor and delivery nurse where she was actively involved in bereavement care. She completed her Master of Science in Nursing degree through Frontier Nursing University in 2021 and began her APRN career in palliative medicine with the adult inpatient palliative care service at Upstate in 2022. She has since obtained her Advanced Certified Hospice and Palliative Nurse certification and hopes to continue to grow and prosper as a provider in this field.

  • Kaylin Brainerd, MSN, DNP, FNP-BC, ACHPN

    Kaylin Brainerd is a certified Family Nurse Practitioner with additional certifications in Advance Certified Hospice and Palliative Nursing as well as Adult Critical Care Nursing, who began working with Family Care Medical Group's outpatient palliative care team in November 2023. She began her nursing career at Upstate University Hospital after completing her Associated degree from Onondaga Community College in September 2013. She spent a majority of her bedside nursing career in the Medical Intensive Care unit while she completed her Bachelors in Nursing through Keuka College and later completing her Family Nurse Practitioner Degree through Upstate's College of Nursing in 2019. Kaylin began her Nurse Practitioner  career in October 2019 with Upstate University's Adult Inpatient Palliative Care team. During her time with Upstate's Palliative Care team she went on to complete her Doctorate of Nurse Practitioner degree through The University at Buffalo where she focused her research on the impact palliative care has on hospital length of stay and 30-day remission rates in adult lung cancer patients. In addition to her multiple nursing degrees, Kaylin also holds a bachelor's in Psychology from LeMoyne College .

  • Caitlin Tomko, MSN, DNP, FNP-C ACHPN

    Caitlin started her nursing career in the critical care setting where she discovered her passion for palliative care. She has worked in Palliative Care for 6 years ever since becoming a nurse practitioner. She has worked in both the inpatient and outpatient setting. She holds a specialty certification as an Advanced Certified Hospice and Palliative Nurse. When she is not working, Caitlin enjoys watching the Buffalo Bills and hiking. 

  • Amit Dhamoon, MD

    Amit grew up in Syracuse, NY. He completed his B.A. in Biology at Williams College in western Massachusetts. He graduated from the combined MD, PhD program at SUNY Upstate Medical University and moved to Boston, MA to complete his internal medicine residency at Massachusetts General Hospital. Amit is the medical director of the palliative care program for Family Care Medical Group in Syracuse, NY. He is committed to delivering high quality, patient and family-centered care in partnership with the excellent providers in the Family Care Medical Group.
